The frontal and lateral radiographs (images A and B) show a soft tissue density in the anterior mediastinum (image A, red arrow), with
obliteration of the retrosternal clear space (image B, red arrow). Correlated axial CT imaging shows a heterogenous mass is seen in the anterior mediastinum (images C and D, yellow arrows), including calcifications (image C, orange arrow).
Anterior Mediastinal Neoplasms
• Teratoma
Can have variable appearance
Mature teratomas show all three tissue densities: lipid, soft tissue, and calcification
• Thymic epithelial/thymoma
Often have cystic components as well as calcifications
Can be difficult to distinguish from a lipid-poor teratoma
• Lymphoma
Relatively homogenous soft tissue mass
Calcifications can be present if previously treated
Often does not displace vessels
• Thyroid
Direct extension or metastatic deposits
